claim form
the form in which one starts civil proceedings in England and Wales under the Civil Procedure Rules. It is worth saying in a dictionary that this represents a deliberate attempt to introduce plain English into legal usage and has accordingly done away with a good number of 'hard words'.

claim form
Formerly known as a writ or originating summons. The form on which a court claim is issued.
In employment law, a form produced by the employment tribunal service for claimants to use to provide the mandatory information relevant to their employment tribunal claim. Since 6 April 2005 claimants are required to use this form to initiate claims in the employment tribunal. Also known as a form ET1.
