A questionnaire prescribed under section 65 of the Race Relations Act 1976 that those who believe they may have been discriminated against on racial grounds can serve on the alleged discriminator. The aim is to obtain information about the treatment in question or the general treatment of a particular racial group in order to decide whether or not to bring legal proceedings before an employment tribunal; and if proceedings are brought, to present their complaint in the most effective way.
